How to Change Address, Phone Number, and/or Email Address

Address changes: We require proof of residency at your new address before we can change it in our system.
Please email your Proof of Residency documentation along with the name of your student(s) to [email protected] or fax 231-794-6416 Attn: Lisa Bezjian, Office Administrator. You may also mail it to: MGLVA, 50 Filer St Ste 324, Manistee MI 49660.
Your address will not be changed in our system until your Proof of Residency is received.

● valid driver’s license
● valid nondriver’s license
● current utility bill (gas, water, electric, sewage, cable, and land line phone)
● current mortgage statement
● current residency card
● deed, vehicle registration
● property tax bill
● current credit card bill
